Insulin Resistance: The Condition Your Doctor Probably Hasn’t Mentioned Yet

Every time you eat carbohydrates, your blood sugar rises. Your pancreas responds by releasing insulin — a hormone whose job is to unlock your cells so they can absorb that glucose and use it for energy. Under normal conditions, this system works elegantly: sugar rises, insulin responds, cells absorb, sugar returns to baseline.

Insulin resistance is what happens when that lock stops working properly. Your cells stop responding to insulin’s signal the way they should. The pancreas compensates by releasing more insulin. For a while, this keeps blood sugar in a normal range — so your annual lab looks fine. But insulin itself, now chronically elevated, begins causing damage of its own.

Key insight: Elevated insulin — not elevated blood sugar — is the earliest measurable marker of metabolic dysfunction. It typically precedes a prediabetes diagnosis by 10 to 15 years. Standard labs don’t measure it.

A routine annual physical typically includes a fasting glucose and an HbA1c — measures of blood sugar, not insulin. These are useful, but they catch the problem late. By the time fasting glucose climbs above 100 mg/dL, insulin resistance has often been present for a decade, and the pancreas has been working overtime for years to compensate.

The markers that catch it early are fasting insulin, HOMA-IR (a calculated ratio of insulin to glucose that quantifies resistance directly), and the triglyceride-to-HDL ratio. None of these are on a standard panel. None require a specialist to order. They simply require a physician who knows to ask for them.
